Demonstrates knowledge and skills necessary to provide services based on the physical,
psychosocial, educational, safety, and other related criteria appropriate population in his/her
assigned area
Abstracts quality data from EHR, validates data prior to reporting, and reports data
to regulatory bodies before deadlines.
Knowledge of the requirements of the Quality Indicator
program required under Centers for Medicare and Medicaid.
Collaborates with providers, nurses, and
other clinical staff to ensure they have a working knowledge base of the Quality Indicators (Core
Measures-E- Measures)
Electronically participates in discharge planning process via feedback to case
managers and nursing concerning Quality Indicator/Core Measure Compliance.
Participates in core
measure implementation and improvements.
Participates in modification of EHR to provide better
outcomes/meet best practice standards as it relates to the evidenced based practice indicator for
the CMS quality Indicator Program.
Utilizes innovative methods to facilitate development and
implementation and compliance with the Quality Indicator requirements
Makes self-available as
resource to nursing staff and providers. Provides learning experiences to strengthen and expand the
staff’s expertise.
Provides feedback to departmental managers on individual staff member’s staff
member performance as available. If individual needs are identified, documents and refers in timely
manner to department manager, individual, or group.
Provides education on selected disease/condition
management as needed by identifying trends of readmission, high risk patients etc.
Serves as
Administrator for Quality Net and updates all key staff on changes and
notifications.
