Job Description
We are seeking a compassionate and dedicated Outpatient Mental Health Nurse Practitioner (PMHNP) to
join our growing behavioral health team at Sumner Regional Medical Center. This role will primarily
focus on providing outpatient mental health services to geriatric and adult patients who have been
recently discharged from our psychiatric inpatient units. As the practice expands, you will also
care for patients referred by our primary care providers.
Key Responsibilities:
Provide
comprehensive outpatient psychiatric care to geriatric and adult patients post-discharge from
inpatient psychiatric units.
Conduct psychiatric assessments, diagnose mental health conditions, and
develop individualized treatment plans.
Collaborate with primary care providers and other healthcare
professionals to ensure seamless continuity of care.
As the practice grows, manage a broader range
of outpatient cases referred by internal providers.
Monitor patients' progress and adjust treatment
plans as necessary.
Educate patients and their families on mental health conditions and treatment
options.
Qualifications:
Current certification as a Psychiatric Mental Health Nurse Practitioner
(PMHNP)
Active state licensure as an NP (or eligibility for licensure)
Experience in providing
mental health services to geriatric and adult populations preferred
Strong communication and
interpersonal skills
Ability to work collaboratively in a team-oriented environment
