Care Coordinator

Our new Team-Based model of care program is a pilot program aiming to support transition aged youth (TAY) with foster care experience attain mental health support, housing stability, employment and education gains, and community connections. This new program provides community-based therapy, case management and care coordination, and peer and family support through a team-based model. The whole team collaborates on the whole caseload, providing youth with an entire team of support, and allowing staff to share in the successes and challenges of their clients.

The Intensive Care Coordinator (ICC) provides support and guidance to family and community members who are a source of strength for the youth. They help bring other providers and the youth’s natural supports together. This position is responsible for engaging and assisting foster youth and other clients, family members, foster parents, and other support persons in forming a Child and Family Team (CFT). The CFT works with identified support persons from natural supports (friends, extended family, neighbors, community members) and formal system partners (child welfare, education, juvenile probation, etc.) to comprehensively address the child and family’s identified goals. 

We currently have two open positions:

1. The ICC coordinates, monitors and contributes to the activities of all parties involved in the care and treatment of the child and family to support and ensure successful and enduring change consistent with the CFT Plan. They also provide a diversity of care through conducting short term needs and strengths assessments and providing stabilization support services to children and TAY in a milieu environment. An ideal candidate demonstrates flexibility, initiative, creativity, inclusivity and knowledge of the unique challenges youth and young adults face in foster care.

2. The Family Intensive Care Coordinator (ICC) draws on their lived experience to provide support and guidance to family and community members who are a source of strength for the youth.They help bring other providers and the youth’s natural supports together. Because of their lived experience, they help create a connection with family and other close contacts, and offer a unique perspective to the rest of the team. The Family ICC is responsible for engaging and assisting clients, family members, foster parents, and other support persons in forming a Child and Family Team (CFT). The CFT works with identified support persons from natural supports (friends, extended family, neighbors, community members) and formal system partners (child welfare, education, juvenile probation, etc.) to comprehensively address the child and family’s identified goals.

• Outreach to and engagement with youth; being a strong advocate for the youth within the team
• Provide community-based trauma-informed services to foster youth or former foster youth transitioning to or struggling with independent living. 
• Provide intensive skill building with youth, with a focus on independent living.
• Provide assessments, crisis intervention, stabilization, and safety plans. 
• Identify and connect youth & young adults with other agencies to address their needs, including housing, medical, mental health, education, employment, advocacy, placement stability, and enrichment activities. 
• Work collaboratively in partnership with the team to engage and connect clients to appropriate resources and services.
• Assist with initial contact and intake, establishing relationships and building trust in order to help clients navigate systems for access to services.
• Provide advocacy, targeted case management and community based rehabilitation services to children and their families. 
• Provide services and participate in agency meetings in a combination of environments including virtual (zoom / phone) and in-person office or community locations (drop-in center, client homes, school, parks, etc.)
• Provide stabilization support services to children and TAY in an in-person milieu environment.

For Family ICC: Direct, lived experience with foster care, social services, juvenile justice, mental health, residential treatment or group home placements, wraparound services, or other related community resources.

For Family ICC and ICC:

• Bachelor's degree in Psychology, Criminal Justice, Social Work or related field (preferred)
• Experience providing case management and rehabilitation services.
• At least 2 years experience working with system-involved youth.
• Demonstrated ability to work with families who have experienced high levels of trauma and demonstrate high mental health needs.
• Ability to work independently and collaboratively as part of a multidisciplinary team.
• Proven ability to be organized, manage time well, complete high quality and timely documentation.
• Clean CA DMV record and current car insurance and car; travel is required.

• Demonstrated experience working with children and families who may be involved in the child welfare system.
• Case management skill – assessment/evaluation, crisis intervention, ability to use supervision effectively, ethical and legal responsibilities, high quality documentation.
• Cultural humility - knowledge of issues of race, class and ethnicity.
• Efficiency and time management - ability to be well-organized and complete timely documentation.
• Collaboration and team functioning - able to work independently and collaboratively as part of a multidisciplinary team.
• Ability to work with clients who have complex traumatic histories and other attachment difficulties.

Position details and compensation: This is a full time, hourly (non-exempt) position. Salary starting at $65,000 per year (+10% for bilingual Spanish) and includes performance-based compensation plan, competitive benefits package, and 403(b) plan.
