Additional Information About the Role

  • Shift Information:  Monday – Friday 8 AM – 4:30 PM (+ 4 weekends per year Saturday and Sunday 8 AM – 4:30 PM)
  • Home Office Location:– BJC Home Care – 670 Mason Ridge Center Dr Ste 300 –Saint Louis, Missouri 63141 
  • Client Territory: This position will cover the Chesterfield and St Charles area as part of the West County Pod Team
  • Case Load: You will be responsible for a caseload of 35–45 patients in patients residences, nursing homes and assisted living facilities.
  • Job Responsibilities: Meeting with patients/families, determine resource needs, determine social support needs, counseling support needs, admission and discharge paperwork, Medicaid and insurance assistance and veteran benefit eligibility, etc.
  • Preferred skills or required work experience:  home health, hospice or palliative experience is not required, but a bonus.  Additionally, this is a very independent position, prior social work experience is preferrable.

BJC Home Care offers patients and their families a complete range of home care services, including skilled nursing services, adult and pediatric hospice and supportive care, rehabilitation therapy, home infusion therapy, infusion treatment rooms, home medical equipment and high–tech respiratory care. Specialty home care programs also are available, including adult and pediatric asthma, cardiac, diabetes, orthopedic and wound care programs.


BJC Home Care provides care to thousands of patients in both Missouri and Illinois. Serving more than 25 counties, it has become the largest home care network in the region and one of the largest in the country.

 

 

Hospice, the final stage of BJC's continuum of care, is a special kind of caring for patients with a life–limiting illness. Services are provided in the comfort of the patient's home or skilled nursing facility. The Hospice staff are sensitive to the physical, psychosocial, emotional and spiritual needs of terminally ill adult and pediatric patients and their families. We provide a multi–disciplinary team of healthcare professionals and volunteers, specially trained in symptom management, pain control, counseling and bereavement services for the dying. Our Hospice services include alternative therapies such as music, art and massage therapy. Our Hospice programs provide palliative care by helping patients manage their pain and symptoms while living their lives with daily peace and dignity.

Role Purpose

Under direction of a physician, assists patients and their families in their residences to adapt and plan an interdisciplinary approach to treating the patient–family system.

 

Responsibilities

  • Consults with patients, families and staff regarding goals of care, end of life issues and palliative needs.
  • Provides direct clinical services to patients and families.
  • Plans and implements social work services on assigned clinical service or unit in collaboration with the healthcare team.
  • Collaborates with community human service agencies to meet the needs of patients, families, and the hospital.
