Job Summary
We are seeking a dedicated and compassionate Registered Nurse Case Manager to join our healthcare team. In this role, you will be responsible for coordinating patient care, ensuring that patients receive the appropriate services and support throughout their treatment journey. Your expertise in emergency medicine, hospital medicine, and patient service will be essential in providing high-quality care to our patients. You will work closely with multidisciplinary teams to develop and implement individualized care plans while adhering to HIPAA regulations.
Responsibilities
• Assess and evaluate patients' needs through comprehensive health history and physical examinations.
• Develop, implement, and monitor individualized care plans in collaboration with healthcare providers and patients.
• Coordinate patient services across various settings including inpatient, clinic, and ICU environments.
• Maintain accurate medical records and documentation in compliance with HIPAA standards.
• Provide education and support to patients and their families regarding treatment options and health management.
• Facilitate communication between patients, families, healthcare providers, and community resources.
• Monitor patient progress and adjust care plans as necessary to ensure optimal outcomes.
• Participate in quality improvement initiatives to enhance patient care services.
Qualifications
• Current Registered Nurse (RN) license in the state of practice.
• Strong knowledge of physiology and medical terminology relevant to nursing practice.
• Experience in emergency medicine or hospital medicine preferred.
• Familiarity with inpatient care processes and clinic operations.
• Excellent patient service skills with a focus on compassionate care.
• Ability to manage multiple priorities effectively in a fast-paced environment.
• Strong communication skills for effective collaboration with multidisciplinary teams.
• Previous experience with medical records management is a plus.

Pay: $83,782.00 - $87,905.00 per year
