?? Location: Puyallup, Washington ?? Job Type: Contract – 13 weeks (Possible Extension) ?? Schedule: Monday, Tuesday, Thursday, and Friday | 07:00 AM – 05:00 PM ?? Pay: $45.00 – $55.00/hour (Based on experience & credentials) Responsibilities: ? Coordinate Patient Care Plans – Develop, implement, and oversee individualized care plans, ensuring alignment with patients' medical needs and treatment goals ? Serve as a Liaison – Act as the primary point of contact between patients, families, healthcare providers, and support staff to facilitate seamless communication and care transitions ? Monitor Patient Progress – Assess patients' conditions, track treatment effectiveness, and collaborate with medical teams to adjust care plans ? Educate Patients and Families – Provide guidance on medical conditions, treatment options, medications, and post-discharge care to promote health literacy and adherence ? Ensure Compliance and Quality Standards – Adhere to healthcare regulations, institutional policies, and best practices to maintain high-quality patient care and safety ? Facilitate Access to Resources – Assist patients in navigating healthcare services, including referrals, insurance coordination, and access to community support programs Required Qualifications: ?? Graduation from an accredited school of nursing ?? Active and unrestricted Washington State RN license ?? Basic Life Support (BLS) certification (AHA) or CPR/AED for the Professional Rescuer (American Red Cross) ?? Minimum 3+ years of experience in Family Practice ??
