Job Description

Job Summary:
Analyzes medical records. Identifies documentation deficiencies. Performs medical record review activities. Summarizes and reports results of review activities to hospital administration, medical staff and Health Information Committee.

Essential Responsibilities:

  • Analyzes inpatient and designated hospital-based outpatient records for documentation deficiencies. Follows medical staff by-laws, reimbursement requirements and The Joint Commission standards relative to completion of medical records. Identifies deficiencies and discrepancies.

  • Re-analyzes records upon completion of deficiencies by medical, nursing and ancillary staff.

  • Notifies practitioners on delinquent/incomplete records. Ensures timely completion of medical records.

  • Performs medical record reviews. Summarizes and reports results of reviews to hospital administration, medical staff, and Health Information Management Committee.

  • Assists physicians with chart completions when necessary.

  • Participates in inservice meetings. Recognizes areas for improvement and skill enhancement.

Basic Qualifications: Experience

  • Knowledge of routine hospital procedures.

Education
  • Post high school coursework in medical terminology, medical records or equivalent related experience.
License, Certification, Registration
  • N/A
Additional Requirements:
  • Demonstrated knowledge of and skill in data analysis, customer service, oral communication, problem solving and teamwork.
  • Demonstrated knowledge of and skill in database PC applications.
Preferred Qualifications:
  • One year medical records experience.
