Practice/Organization Details:
•  Provide exceptional care to patients in a hybrid in-home and virtual model
•  In-Home is mostly Medicare annual assessments with the ability to work independently
•  Manage a low-volume patient panel to ensure a focus on quality of care
•  Established clinical care team including 2 experienced physicians, 10 APPs, pharmacist, nurses, and more
•  Flexible schedule to work around your needs throughout the day - supreme work/life balance offered

Compensation & Benefits:
•  Strong salary range of up to the 70th percentile of MGMA - Anywhere between $120-$150K
•  Full benefits package including medical, dental, vision, 401k, and more
