Nurse Practitioner

65 Prime Plus by Mercy is seeking Nurse Practitioners across Missouri, Oklahoma and Arkansas with an interest in building relationships, caring for patients with complex needs, changing lives, and making a difference for seniors. This is an outpatient-only position, no call position with patients on Medicare and Medicare Advantage.  Our 65 Prime Plus by Mercy team consists of 2 Physicians, 2 Advanced Practitioners, and an engaged support staff focused on quality patient care.  

65 Prime Plus by Mercy highlights:  

• Value-based care – adequate time with patients, relationship building, quality incentives. 
• 30 minute – 1 hour appointment slots for all patients – adequate time without rush  
• Patient panel will offer intellectually stimulating and rewarding environment
• Dedicated panel nurse for telephone encounters, triage, and in basket support 
• In-person, virtual, or telephonic encounters – annual comprehensive visits, transitional care, and same day visits 
• Exceptional work/life balance
• Provide team-based care to manage a panel of 1,000 with collaborating physician allowing for longitudinal relationships with providers, patients and families
• No holidays, weekends, or evenings
