Job Description
The Pueblo, CO Veterans Affairs Outpatient Clinic is recruiting primary care physicians.
Our Primary Care Provider functions as a team provider in a shared workflow with a multidisciplinary Patient-Aligned Care Team (PACT - a medical home model). The PACT team model consists of an MD (or PA/NP), RN, LPN, and Medical Support Assistant. Provide leadership to the team including shared delegation of appropriate care and care processes to appropriate team members. You will be assigned to provide comprehensive, collaborative same-day and continuity care to an assigned panel of Veterans via face-to-face or telehealth modalities utilizing all available tools, such as registries, to enable effective population health management. PACT Providers are expected to demonstrate excellence in customer service via compassionate and informed interpersonal relationships with Veterans, family members, and staff. Participate in team performance improvement and sustainment activities to optimize team efficiency and care delivery to patients.
Sign on bonus is available. Assistance with payment of med school loans up to $200K may be available.
