We are looking for a Remote Care Manager RN - AIM (Advanced Illness Management) Program - to join our team, covering our Clinton Township/Detroit branch. This position will directly report to the Director of Care Management AIM and is responsible for working with patients, their families, PCP, IDT, and other health care professionals to provide and coordinate high-quality end of life care.

• 2 years of prior skilled Registered Nurse experience
• Active acute unencumbered state RN license - Michigan RN Licensure
• Must be willing and able to be licensed as an RN in all states applicable to role
• Ability and interest to provide direct patient care in homes and facilities
• Demonstrated experience developing and maintaining rapport with patients and families, and ability to work well with an interdisciplinary team
