Overview:
We are looking for a Remote RN Care Manager - AIM (Advanced Illness Management) Program to join our team, covering the Twin Cities, MN branch. This position will directly report to the Director of Care Management AIM and is responsible for working with patients, their families, PCP, IDT, and other health care professionals to provide and coordinate high-quality end of life care.
About You:- 2 years of prior skilled Registered Nurse experience
- Active acute unencumbered state RN license - MN RN Licensure
- Must be willing and able to be licensed as an RN in all states applicable to role
- Ability and interest to provide direct patient care in homes and facilities
- Demonstrated experience developing and maintaining rapport with patients and families, and ability to work well with an interdisciplinary team
