  • Proof of vaccine series or completion and approval of client medical or religious exemption form. Preferred, not required.
  • Positive MMR titers ONLY from within the last 15 years.
  • Completed within 1 year of start updated annually.
  • Will accept proof of full Hepatitis B series, or at least two vaccines from the series completed.
  • If positive, CXR within 5 years and current Questionnaire.
  • Positive Varicella titers ONLY from within the last 15 years.
  • Proof of vaccination within past 10 years, declinations not accepted.
  • If 2 step PPD not done within past year.
  • Agency to provide completed I-9 or attestation that completed I-9 is on file.
  • If not ran with background check, to be complete within 1 year of start.
  • Live in Ohio for 5 years: Ohio Fingerprints (BCI&I check) AND FCRA-Compliant National Database Criminal background check.
  • Highest level of education verified.
  • Must provide verification of employment within 5 years.
  • Clinical Only: Skill specific required to be complete within 1 year prior to start.
  • Competency Exams for the relevant department. Must score at least 80%.
  • 80% passing score required, Completed within 1 year prior to start.
  • Physical Exam completed within one year of initial start date.
  • Proof of vaccine from Oct-March or exemptions must be submitted for approval.
  • Non-expired photo ID provided.
  • 2-Step TB test with negative reads within the last 6 months.
  • Resume to include Education and Work History, 2 Professional References.
  • Shoulders up, plain background, headshot.
  • Ran within 6 months of Start. Search includes SSN Trace.
  • Verified to be ran separately from background check.
  • Supporting documentation that the individual was screened and cleared.
  • Ran within 60 days upon initial start date to include the following drugs: Amphetamines, Barbiturates, Benzodiazepines.
  • Safety and Compliance Training modules emailed to agency POC by UHS after acceptance.
  • For Ohio residents to meet fingerprint requirements, please upload FCRA-Compliant National Database Criminal background check.
