Role Purpose

The Value–Based Care Consultant (RN) works directly with value–based care (VBC) performance leadership on all set organizational quality and utilization goals as it relates to value–based contracts. This position is responsible for engaging providers and offices in a data driven approach to implement and optimize value–based care workflows to improve population health management efforts, including cost, quality, access, network optimization, and documentation and coding.
