Job Title: Clinical Transition Educator Job Summary: The Clinical Transition Educator plays a crucial role in ensuring patients' successful transition to Option Care Health's (OCH) care delivery model. This position involves providing clinical education and training to patients, caregivers, and facility staff, as well as collaborating with the sales team to increase the number of patients transitioning to OCH services. Key Responsibilities: • Patient and Caregiver Education: Evaluate, educate, and train patients, caregivers, and facility staff on how OCH services and products will be delivered in alternative sites, homes, or virtually. • Patient Assessments: Conduct assessments to determine patients' suitability for transitioning to the OCH care model. • Collaboration: Work closely with the OCH sales team to identify educational needs and develop strategies to increase patient transition volume. • Confidentiality: Ensure the confidentiality of patient and proprietary information, adhering to legal guidelines and OCH policies. • Documentation: Properly document all education and training activities using OCH applications and technology. Basic Qualifications: • Active and unrestricted Registered Nurse (RN) license. • Minimum of 2 years of experience in the healthcare industry. • Strong relationship-building skills with individuals at all levels of an organization. • Knowledge of relevant standards, including ACHC and URAC. • Excellent customer service skills and experience in evaluating customer satisfaction. • Proficient in using the internet, email, Microsoft Word, Excel, and PowerPoint. • Experience in identifying operational issues and implementing process improvements. • Reliable transportation for travel to care facilities, home visits, and hospitals. If using a personal vehicle, a valid driver’s license and proof of insurance are required. • Willingness to obtain nursing licensure in additional states as needed. • Strong presentation and organizational skills. Travel Requirements: • 100% local travel to and from partnership facilities, community hospitals, and medical practice offices to promote Option Care Health services, process referrals, and provide education and training support to patients, caregivers, and referral sources. Preferred Qualifications: • Hands-on experience in home or alternate site infusion or discharge planning. • Experience in growing service provider partnerships.
