Job Description
The Senior Program Manager is responsible for the strategic oversight, implementation, and coordination of both HIV treatment and prevention programs at Wyckoff Heights Medical Center. This includes supervision of staff across multiple HIV Case Management/Treatment grant-funded programs and new and existing HIV prevention services, including PrEP, PEP, STI screening, education, and community outreach. The Senior Program Manager ensures the integration of evidence-based practices, quality improvement initiatives, and community partnerships to support individuals at risk of or living with HIV.
β’ Program Leadership & Supervision.
β’ Oversee the implementation and fidelity of all HIV treatment and prevention programs.
β’ Supervise and support a multidisciplinary team, including specialists, peer educators, and prevention outreach staff.
β’ Coordinate regular case conferences and team meetings.
β’ Provide staff training and individual supervision to support high-quality service delivery.
β’ HIV Prevention Program Oversight.
β’ Lead the planning, execution, and evaluation of HIV prevention initiatives, including PrEP/PEP education and access, harm reduction, STI prevention, and testing.
β’ Develop outreach strategies for high-risk populations and underserved communities.
β’ Collaborate with community-based organizations to increase awareness and engagement in prevention services.
β’ Quality Improvement & Reporting.
β’ Ensure programs meet established performance indicators, including linkage to care, viral load suppression, and prevention targets.
β’ Monitor data collection, ensure timely and accurate reporting, and analyze outcomes for continuous program improvement.
β’ Prepare and submit monthly and quarterly program reports to internal leadership and funders.
β’ Client Care & Case Management.
β’ Maintain a small caseload, providing comprehensive services including behavioral health assessments, crisis intervention, and referral coordination.
β’ Train and supervise staff on motivational interviewing, evidence-based prevention strategies, and trauma-informed care.
β’ Community Engagement.
β’ Serve as a liaison for the Consumer Advisory Board (CAB), supporting recruitment and facilitation.
β’ Represent the organization in community forums, health fairs, coalitions, and Department of Health initiatives.
β’ Develop and disseminate educational and promotional materials to raise awareness of HIV prevention and treatment resources.
Position requirements:
β’ This position requires a minimum of a masterβs degree and a minimum of 3 years of job-related experience in a supervisory position.
